In this case study, a novel intravitreal hydrogel depot, Rab&Rapa-M@G, was developed to deliver both ranibizumab (Rab) and rapamycin (Rapa-M), an mTOR inhibitor, in a sustained release formulation. The hydrogel, consisting of hyaluronic acid (HA) and Pluronic F127, serves as a thermosensitive matrix that allows for the co-delivery of ranibizumab and rapamycin for at least 14 days following a single intravitreal injection. This dual-drug delivery system effectively targets the retinal pigment epithelium, improving retinal autophagic flux homeostasis and mitigating oxidative stress injury in age-related macular degeneration (wAMD) mice. The formulation modulates key signaling pathways, including AMPK/mTOR/HIF-1α/VEGF and AMPK/ROS/HO-1/VEGF, to enhance retinal function. This study highlights the synergistic potential of ranibizumab in combination with rapamycin, demonstrating significant improvements in retinal health, and offers a promising therapeutic approach for sustained ocular drug delivery in retinal diseases.
